Cost

$100 – Application fee (includes license and background check)
$40 – Fingerprint Processing Fee
$50 – $100 – Initial License Fee
$15 – $20 Passport Photo Fee
$20 – Transfer MBLEx results to Oregon Board (Non-Applicable if you indicated to
- send results to Oregon on your original FSMTB application)
$15 – Original Transcripts sent with application from applicant to Oregon

Exam Required

Pass the MBLEx OR NCBTMB Exam (have results sent to Oregon)
- Complete MBLEx Result Transfer Form to have proof of test sent to Oregon
- Pass the National Certification Board for Therapeutic Massage and Bodywork
Complete the Oregon Jurisprudence Examination
- Applicant will receive a link and log-in after acceptance of application

Minimum hour requirement of 625 hours
