$5-$20 Passport photo fees
$20 Fee to have MBLEx results from MBLEx examination taken in another state transferred.

Exam Required

Pass the MBLEx Exam (have results sent to Connecticut)
Complete the MBLEx Reults Transfer Form to have proof of test sent to Connecticuit
Pass the National Certification Examination for Therapeutic Massage and Bodywork

Hours Required

Minimum hour requirement of 500 hours
